McHenry Celiac Walk 2012

If you do not have time to send in your registration,

just bring it with you the day of the walk!

Join us Saturday, May 19, 2012 and help to raise money

for the University of Chicago Celiac Disease Center.

The walk will be held at Emricson Park, Woodstock, IL.

Sign-In begins at 9AM and the walk will start at 10AM. No fee to participate!

There is a 1.3 mile and a 1.7 mile trail. Go around once, twice, as many times as you want!

Go to: www.cureceliacdisease.org/donate-now and make your Pledge Payment now!

Please put "Woodstock Walk 2012" in the Additional Instructions box at the bottom of the page.

Please print and fill out registration form, print and sign the

waiver and release form, and mail both to:

Print the Pledge Form and bring to the walk - turn in when you sign-in the day of the walk

Checks should be made out to: University of Chicago Celiac Disease Center

If unable to walk, mail pledge sheet and checks to:
